I, the undersigning applicant of Santo Rico Dance, Inc, acknowledges that I am applying for instruction in Latin Dance, or any variation therefore, involving physical activity. I acknowledge that the school carries no insurance against injury to any of its students. As a condition to being admitted to the school as a student, I assume the risk of all injury and hold Santo Rico or its instructors and agents harmless from any and all liability (including attorney's fees and costs) for all claims, actions or damages, due to injuries suffered by me or caused to third parties by me arising out of activities involving Latin Dancing, or any variations therefore, whether occurring on the premises of the school or elsewhere, excepting only those claims, actions, or damages caused by the gross negligence or intentional act or mission of any of them.

We are not responsible for lost or stolen property and/or membership passes.

All entry-level applicants are subject to a $25 Registration Fee used toward the purchase of the school's basic level instructional video.

Upon completion, this form will be kept on file at our studio, where you will receive your personal copy of our instructional DVD upon your 1st visit to take classes. Feel free to print out a copy of your order form as proof of purchase and for your records

Yo, el solicitante a Santo Rico Dance, Inc., reconozco que estoy solicitando instrucciones de baile latino, u cualquier otra variedad disponible, cual requiere actividad física. Yo comprendo que la escuela no carga seguro contra algún daño físico que le incurra a algún estudiante. Como condición a hacer admitido a la escuela como estudiante, yo asumo toda responsabilidad de daño físico y Santo Rico o sus instructores o sus agentes no son en ninguna circunstancia responsables ( incluyendo tarifas y costos de abogados), en todo reclamo, acción o daños, que pueden ocurrir o sufrir a mi, o por causa de una tercera persona, al yo estar hacienda actividades cual involucra baile latino u otra o cualquier otra variedad, ocurrida en el establecimiento de la escuela o en otro lugar, excepto esos clamando acción o daños causados por la negligencia o acto intencional o misión de cualquier otra persona.

No somos responsables de ningún articulo/propiedades/membresias perdidas.

Todo solicitante de nivel principiante es sujeto a pagar $25 de registración, cual es usado como pago para obtener el DVD de nivel básico.

Al completar este formulario su información sera transmitida a nuestro estudio donde se le dará su copia de nuestro DVD instrucciónal cuando se presente por primera vez a tomar clases. Puede imprimir una copia de su recibo de compra para sus datos, y para prueba de compra
